Medical Certificate for infant (at birth) Application

Midori Ward also accepts applications for grants from Medical Certificate for infant (at birth) to the following individuals.
◆For more information about the Subsidies for Medical Expenses for Pediatric Treatment business, please visit the City of Yokohama, Health and Social Welfare Bureau National Health Insurance page.

Person who can grant application by mail

◆Those who have resident certificate in Midori Ward and have social insurance (employee health insurance other than Yokohama National Health Insurance)

※After the application is accepted, we will send Medical Certificate for infant to your home by regular mail.

(If you are a member of the Yokohama National Health Insurance, you will need to apply for your child at the 26th floor (National Health Insurance Section) on the 2nd floor of the ward office, then issue Medical Certificate for infant at the 25th window (National Health Insurance Section payment charge))

Method of mail application

  1. When you submit birth registration form, please drop in at the 25th window on the second floor of the ward office (National Health Insurance Section Payment Section). To apply, you need to check your child's participation in employee health insurance. If your child's name is not listed on your health insurance card, you will not be able to issue Medical Certificate on the day. We hand over the application form and explain the system, such as the medical expenses before the issuance of Medical Certificate.
  2. You can receive or download the application form at the 25th window (National Health Insurance Section payment charge) on the second floor of the ward office. ※Please print the application form in A4 size. Also, please refer to the entry example and fill out and seal the required items.

  3. Please send a copy of your application and your health insurance card with your child's name in a sealed letter.

Please note that applications will not be accepted if there are any missing items, missing seal, or missing attachments. You do not need a reply envelope.
