Application for mail delivery of Medical Certificate for infant (at birth)

Midori Ward also accepts applications for granting Medical Certificate for infant (at birth) by mail to the following persons:
◆For more information about the Subsidies for Medical Expenses for Pediatric Treatment business, please visit the City of Yokohama, Health and Social Welfare Bureau National Health Insurance page.

Those who can apply for grant by mail

◆Those who have resident certificate in Midori Ward and have social insurance (employee health insurance other than Yokohama City National Health Insurance)

※After the application is accepted, Medical Certificate for infant will be sent to your home by regular mail.

(If you are a member of Yokohama City National Health Insurance, you will need to apply for your child at the 26th window on the 26th floor of the ward office (National Health Insurance Section), and then issue the Medical Certificate for infant at the 25th window on the National Health Insurance Section).

How to apply by mail

  1. When you submit birth registration form, please drop by at the 25th window on the second floor of the ward office (National Health Insurance Section Benefits Section). To apply, you need to check your child's enrollment status in employee health insurance. If your child's name is not listed on your insurance card, you will not be able to issue Medical Certificate on the day. I will give you an application form and explain the system of medical expenses before the issuance of Medical Certificate.
  2. You can receive the application form at the 25th window on the second floor of the ward office (National Health Insurance Section Benefits Section) or download it. ※Please print the application form in A4 size. Also, please refer to the entry example and fill out and seal required items.

  3. Please send a copy of your application form and your health insurance card with your child's name in a sealed letter.
