What is a comprehensive community care system?
Japan is aging at an unprecedented rate in other countries, and it is expected that the number of elderly households living alone, married couples alone, and elderly people with dementia will increase rapidly.
Therefore, by 2025, when the baby boomers will be 75 years of age or older, nursing care, medical care, nursing care prevention, and life so that the elderly can continue their own lives in areas where they are accustomed to living as much as possible. It is necessary to promote the construction of a comprehensive support and service provision system "comprehensive community care system" for each area of daily life (generally junior high school district) where support and housing are provided integrally provided.
Midori Ward Action Plan (formerly Action Guidelines) for the construction of a Yokohama-style comprehensive community care system
In fiscal 2017, Midori Ward formulated the Midori Ward Action Guidelines for the Establishment of a Comprehensive Community Care System for Yokohama, and has been promoting initiatives.
Based on the efforts since the formulation of the Action Guidelines and changes in social conditions, we revised the Midori Ward Action Plan for the Establishment of a comprehensive community care system in Yokohama.
