Submission of the examination request

In order to make a request for examination, it is necessary to submit a document (examination request) to the Examining Agency.
When submitting a request for examination, please submit two copies of the copy (Article 4, Paragraph 1 of the Administrative Appeals Examination Law Enforcement Order).

Items on the examination request

The items required for the examination request are stipulated in the Administrative Appeals Law.
Please note that the required items are different between the request for examination for disposal and the request for omission.
Please refer to the form example and description example when preparing the examination request.

Request for examination of disposal (Article 19, Paragraph 2 of the Administrative Appeals Examination Act)

Purpose and reason for the request for examination
The purpose of the request for examination is the purpose of the request for examination.
If the purpose is to cancel the building confirmation disposal, please state "Request a decision to cancel the building confirmation disposal."
The reason for the request for examination is that when requesting the cancellation of the disposition, please state the illegal or unreasonable point of the disposition as specific as possible.
Whether or not there is instruction by the Disposal Agency and its contents
Teaching means that the Disposal Agency (mayor, etc.) presents the petitioner and the period during which the petition can be filed to the other party at the time of disposal.
In the case of a written disposition, it is stated in the disposal notice.
If a third party that is not the other party of the disposal makes a request for examination, please write "None".

Points to keep in mind when submitting an examination request

When multiple persons make a request for examination

A request for examination can be made by multiple people, but in that case, up to three representatives can be determined (Article 11, Paragraph 1 of the Administrative Appeals Examination Law).
The representative is the representative of the requester for examination and can perform any act related to the request for examination other than withdrawing the request for examination (Article 3 of the same Article).
If you have more than one representative or if you do not set a representative, please specify the delivery place for documents.
If the representative is not set, the examination committee may order the representative be elected (Article 2 of the same Article).

Requests for examination by proxy/agent

A proxy/agent may be appointed to request a review (Article 12, Paragraph 1 of the Administrative Appeals Law).
proxy/agent may perform any actions relating to the Requester for Examination.
However, withdrawal of a request for examination can be made only when specially commissioned (Article 2 of the same Article).

When a corporation makes a request for examination

Not only natural people but also corporations (including non-corporate associations or foundations) can request a review.
