Submission of the request for examination
In order to make a request for examination, it is necessary to submit a written document (examination request) to the Examining Agency.
When submitting a request for examination, please submit two copies of the copy (Article 4, Paragraph 1 of the Enforcement Order of the Administrative Appeals Law).

Items to be stated in the request for examination
The items required for the examination request are stipulated in the Administrative Appeals Law.
Please note that the required items are different between the request for examination of disposal and the request for examination of omission.
Please refer to the example format and description example when creating a request for examination.
Request for examination of disposal (Article 19, Paragraph 2 of the Administrative Appeals Law)
- Name or name of the requester and Address or residence
- Disposal pertaining to the request for examination
- Date when we learned that there was a disposition pertaining to the request for examination
- Purpose and reason for the request for examination
The purpose of the request for examination is the purpose of the request for examination.
If the purpose is to cancel the building confirmation disposal, please state "Request a decision to cancel the building confirmation disposal."
The reason for the request for examination is that when requesting cancellation of the disposition, please state as specific as possible the illegal or unjustified points of the disposition. - Whether or not there is any instruction of the Disposal Agency and its contents
Instructions are when the Disposal Agency (mayor, etc.) presents the petitioner for the appeal and the period during which the petition can be filed to the other party at the time of disposal.
In the case of a written disposition, it is stated in the notice of disposal.
If a third party who is not the other party to dispose of requests for examination, please indicate "None". - Date of request for examination
Request for examination of omission (Article 19, Paragraph 3 of the Administrative Appeals Examination Act)
- Name or name of the requester and Address or residence
- Details and date of application for dispositions and other acts relating to the omissions
- Date of request for examination
Points to keep in mind when submitting a request for examination
When a request for examination is made by multiple persons
Requests for examination can be made by multiple people, but in that case, up to three representatives can be determined (Article 11, Paragraph 1 of the Administrative Appeals Law).
The representative is the representative of the requester for examination and can perform any act related to the request for examination other than withdrawal of the request for examination (Article 3, Paragraph 3 of the same Article).
If more than one person is determined or if the total representative is not determined, specify the place of delivery of documents etc.
If the representative is not set, the examination committee may order the representative to be elected (Article 2, paragraph 2).

When a request for examination is made by proxy/agent
When submitting a request for examination, proxy/agent may be elected (Article 12, Paragraph 1 of the Administrative Appeals Examination Act).
proxy/agent may perform any act relating to the requester's request for examination.
Provided, however, that the request for examination may be withdrawn only with a special commission (Article 2, Paragraph 2).

When a corporation makes a request for examination
Not only natural persons but also corporations (including non-corporate associations or foundations) can request examination.
