5-(3) Notification required only for designated NPO corporations

Documents to be submitted annually only by designated NPO corporations

In addition to business reports, the designated NPO corporation shall submit the following documents within the first three months of each fiscal year to explain that the operating organization, accounting are appropriate and that there is no violation of laws and regulations. Must be submitted to

  • Documents describing statements by source of revenue, details of borrowings, and other matters related to funds

B. Transactions with Executive Officers and others

  • Donor (an officer of the designated NPO corporation, spouse of an officer, a relative within the third degree, or a person who has a special relationship with the officer, and the total donation to the designated NPO in the previous fiscal year is 200,000 yen Documents stating the name of the donation and the date of receipt.
  • Documents describing the total number of employees who have been paid and the total amount of salary for the staff
  • Amount of donations spent, their counterparts and date of expenditure
  • Documents describing the amount and use of money when remittances or taking out money overseas, and the date of implementation
