What is physical checkup at school?
- When entering elementary school, physical checkup implements this program in accordance with the School Health and Safety Act to understand the health status of children, provide necessary advice on health, and prepare for admission.
- The physical checkup program is held by municipalities for children who enter elementary schools (including the first half of the compulsory education School and elementary schools of special needs schools) in April of the following year.
- The contents of physical checkup include Internal medicine health check-up, Ophthalmology health check-up, dental examinations by school doctors and school dentists, visual acuity examinations by faculty and staff, hearing examinations and Education Counseling as necessary.
- In examinations and consultations by school doctors and school dentists, we recommend that you detect early if there is any suspicion of illness that may interfere with school life and daily life and consult a doctor.

In fiscal 2024, physical checkup at school (for those who plan to enter elementary school in April 2025) may cancel physical checkup at school on the day due to the effects of natural disasters such as typhoons and infectious diseases.

Implementation venue
You will have a medical examination at an elementary school in the school district designated by each Address.
You will not be able to receive a consultation outside of the designated consultation venue.
Date of implementation
Weekday afternoon from late October to early December (the implementation date and time vary depending on the venue)
Implementation Guide
Eligible families (those who have registered as Address in Yokohama City as of October 1) will receive information about physical checkup at school from the ward office in mid-October, along with admission to municipal school. Please check the venue and date of implementation.
Attention at the time of consultation
Please refrain from visiting the school by private car, motorcycle, or bicycle on the day of the consultation. Also, at school, physical checkup usually takes about 1 hour and 30 minutes to 2 hours. Please note that the scheduled time may be exceeded depending on the number of children and the progress of physical checkup. In addition, please refer to each school or health education, food education section about announcement such as storm warnings with typhoons or conduct by influence such as infectious diseases. It will be posted on this website.
Requests to parents when conducting preschool children physical checkup
- Please bring your letter of attendance at school, physical checkup questionnaire at school, shoes (for children and parents), bags for outerwear (for children and parents), writing utensils, and handkerchiefs. If you are visiting the school, please check the health status of the day before coming.
- If you have a fever (at least 37.5°C), please refrain from attending physical checkup at school.
- If you have a fever (37.5°C or higher) or are unable to have a medical examination due to circumstances, please contact the elementary school at the physical checkup venue. Please note that you will not be able to receive a medical examination at other dates or venues. The absence of physical checkup at school will not interfere with enrollment. Since physical checkup will be held immediately after entering elementary school, individual consultations at medical institutions are not required in principle.
FAQs
Q.I can't see a doctor on the designated day due to circumstances. Can I receive it at another school? A.You cannot go to the designated school. Please have your convenience as much as possible and have a medical examination at the designated school. If you are unable to have a medical examination due to work or poor physical condition of your child, please contact the designated school for absence. There are handouts and announcements for admission.
Q.I can't see you because it's not convenient. Can I enter elementary school? A.Even if you are unable to receive a medical examination, there is no problem with admission. In addition, in principle, there is no need to have a medical examination individually at medical institutions.
Q.I plan to enter a private or national school. Do I need to have a medical examination? A.If you plan to enter a private or national school, please consult a doctor in principle. After that, if you decide to enter a private or national school, please contact the school you visited. The physical checkup vote will be sent from the Board of Education to the school you plan to enroll in. If admission is confirmed before the consultation, the necessity of attending a physical checkup at school may differ depending on the school, so please check directly with the private or national school. As a result, if you do not have a medical examination, please contact the designated school for absence.
Q.We cannot attend on the day due to the inconvenience of parents. A.It's okay for anyone other than your parents, such as grandparents. If you are familiar with your child's condition and health condition, please be accompanied by a person.
Q.Do you need a mother and child handbook? A.You don't need to bring your mother and child handbook.
Q.I lost the "physical checkup Survey Form at School" enclosed with "admission to municipal school". A.The school has spares, so please contact the designated school. If you want to fill in the form on the day, please bring your mother and child handbook as there is a column to fill in the history of Vaccinations.
Q.I plan to move out of the ward (outside the city) during the physical checkup period. A.(If the medical examination in designated school is before moving) At designated school, please. After the consultation, the physical checkup vote will be sent from the school (or Board of Education) to the school you plan to enroll in (or the local government you are moving to).
(If the medical examination in designated school is after moving) Please consult with your local government or local government. At the same time, please contact designated school for absence.
Q.I plan to move in from outside the ward (outside the city) during the physical checkup period. A.(If you can consult at the original municipality before moving in) Please consult your doctor at the original municipality. In that case, there is no need to take a medical examination again after moving in.
(If you cannot have a medical examination at the original municipality before moving in) If you have a medical examination at the school you plan to enroll after moving in, please consult with the school you plan to enroll. If you have completed a medical examination at the school you plan to enroll, you will not be able to receive a medical examination. Even if you are unable to receive a medical examination, there is no problem with admission.
