Yokohama City semi-protection elementary school studenｔ Student Glass

Among children who have been certified by the Subsidies for School Expenses at Yokohama Municipal Elementary School, Junior High School, and compulsory education Schools, children who need glasses due to poor visual acuity.

• Certified by Subsidies for School Expenses
• Naked eye vision in school physical checkup (for elementary school studenｔ students who already have glasses, visual acuity after correction) must be C or less (less than 0.7) in one eye.

The school will give the target child a survey form, so please fill out the form and submit it to the school within the deadline.

• Request survey: May to June ("glasses tickets are issued around mid-July)

• Optical inspection fee (per person) 2,750 yen (uniform)　※Please give priority to the pediatric medical system for eye inspection.　
• Glass purchase price (including adjustment) Maximum amount 5,000 yen (up to one per person)

　　※All of the above amounts include tax.

• The number of assistance (glasses purchase price) by this project is once at elementary school and once at junior high school per person.
• You will not be able to receive assistance without a "glasses ticket".
• Limited to purchases at designated stores after consulting and eye examinations at designated medical institutions.
• If the amount exceeds the limit when purchasing glasses, the difference will be borne by the guardian.
• Purchases and contact lenses due to damage to glasses are not eligible for assistance from this project.

Please fill out the invoice and request it to the Health Education and Food Education Division after filling out the invoice and billing assistance.
