About review of "allergic disease life management instruction table"

(1) Name change
 What used to be a "nursery school" has been changed to "childcare and educational facilities", making it easier to use in kindergartens and other facilities.


(2) Addition of bronchial asthma column
 It was added to understand the situation of the target elementary school student because anaphylactic shock tends to become more serious when anaphylactic shock occurs.
 In addition, when we list only bronchial asthma, we list "only for children who need special consideration and management in life in nursery schools".
 In addition, atopic dermatitis, allergic conjunctivitis, and allergic rhinitis are described only if management is required in the same way, but in that case, use the national style.


(3) Deletion of review of removal period and revaluation
 We delete period such as re-evaluation which we established in style of Yokohama-shi original until now. The national guidelines state that "at least once a year, resubmit the life management guidance table, etc., according to the child's allergic condition." Therefore, the garden asks the doctor to confirm that there is no change in the allergic condition, and if there is no change, no resubmission is required. At that time, the date that the guardian has confirmed with the doctor will be confirmed in the garden and recorded.

We conducted an allergy survey of childcare and educational facilities in elementary school student, and shared the results with Fire Bureau.

Accidents of accidental eating accidents

In the event of an accidental eating accident, we ask the city to submit a “school lunch-related Report”.
We provide the report to each facility and use it to prevent accidents as part of the training in the park.

※The “Removal Cancellation Notification” <Form 5> is a document to be filled out by a doctor and a guardian who receives a cancellation instruction. (In the national allergy guidelines, cancellation instructions do not require the submission of a life management guidance table or a doctor's medical certificate.)

Response to food allergy accidental eating accident

In the event of a food allergy accident, please submit the "school lunch-Related Report (Form 7)" to the ward office Children and Families Support Division.
