Overview of Community Care Plaza
A facility that provides comprehensive welfare and health services in familiar places, accepts consultations, and serves as a place for activities and exchanges related to welfare and health so that everyone can live in the area where they lived. is.
It can be used by anyone, from babies to elderly people.

Main Functions
(1) Regional Activities and Cultural Exchanges
- Rooms such as multipurpose hall, cooking room, community care room, volunteer room, etc. can be used as a place for activities and exchanges such as welfare and health of local residents.
- We hold various events and lectures hosted by Care Plaza. In addition, we provide and coordinate information on local activities and volunteers.
(2) Consultation and support for welfare and health [Community Comprehensive Support Center]
- As a local consultation desk, welfare and health professionals receive various consultations, provide information on The Long-term Care Insurance services, etc., and coordinate with related organizations.
(3) Care prevention support and home care support
- We make use plan of necessary service (care prevention care plan) for elderly people (one who received authorization of support required 1, 2, target person of business). In addition, we perform communication adjustment with home care support company and service company.
(4) Care prevention outpatient care (day service (elderly day care)) ※There is also a community care plaza without day service (elderly day care).
- We provide day-care services that support daily life, such as bathing, eating, gymnastics and recreation.
Konan Ward Initiatives
(1) Watching and cooperating companies
- It is businesses that have you cooperate with gentle watching in the wake of "greetings" and "calling out" in everyday life. Care Plaza calls out to the shops in the town and watches over and expands the circle of cooperating companies.
