Application for road width certification

Please make the same two copies of the "Road width certification request" and submit it.

1. Address, name, Phone number
2. Locations required for certification (item 1 on the surface of "Request for certification")
3. Reason for the need for certification (item 2 on the surface of "Request for certification")
4. Location map near the garage (back side of "Request for certification")

• The lot number of the certificate required is in contact with the public road. ※It may contain a private road.
• If there are more than one lot number, all lot numbers must be entered.

• Please fill in the amount of the fee on the "payment notice and receipt" to hand at the time of reception and pay at the window of the designated financial institution in Yokohama City.
• We will ask you to show your receipt when you receive the certificate, so please do not lose it.
• The fee is 300 yen per case.
