Citizens aged 40, 50, 60, and 70 living in Yokohama as of the date of the consultation.
(Please bring your health insurance card to confirm your identity at the time of consultation.)

① Please make a reservation directly to the desired medical institution by telephone, etc., saying, "I want to receive periodontal disease health check-up in Yokohama".

However, those who fall under the following are exempt from the cost.
・70 years old → Please bring a health insurance card or other items that can confirm your age.
・social security households → Please bring your "Request for Medical Treatment on Holidays and Nights"
・Those who are subject to the Support Benefit Program for Residual Japanese in China → Please bring your "identification card"
・Households that are exempt from municipal tax and prefectural tax in the previous fiscal year or those who are taxable only on a per capita rate → Procedures are required before health check-up. Please contact your ward office, Health and Welfare Division Health Promotion Section.

Yokohama-shi periodontal disease health check-up is performed in the following items.
1． Tooth condition (All teeth will be checked)
2． Periodontal tissue status (measure the periodontal pocket * of specific teeth)
※Periodontal pocket: Groove between teeth and gums

※We do not carry out image inspection or tartar removal by X-ray in Yokohama-shi periodontal disease health check-up.
In addition, even if treatment is required, treatment will not be performed on the same day of health check-up unless the person consulted wishes.
(If treatment is performed on the same day of health check-up, treatment fee will be collected at the counter separately fromhealth check-up consultation fee.)
