From March 10, 2008, the invoice number (*) has been printed in the name of the bank transferYokohama Municipal Citizen's Hospital and Yokohama City Apolexy and Spinal Nerves Center.

※The invoice number is to be assigned an arbitrary number (within 6 digits, only numbers) so that you can confirm the details of the transfer. When submitting an invoice, please fill in the invoice number.

The transfer person's name is "Yokohamashi T XXXXX".

※○○○○○○The part is the invoice number

(Example) In the case of invoice number 123456 ... "Yokohamashi T123456"

Payments by invoices without the invoice number will be printed on the passbook as follows.

• Payment of municipal hospital ... ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・
• Payment by stroke / neurovertebral center: "Nosotsuchiyushinke"
