Payment of transaction price

Thank you very much for your understanding and cooperation in the operation of Yokohama Municipal Hospital.

With regard to the payment of transaction fees for Yokohama Municipal Citizen's Hospital and Yokohama City Apolexy and Spinal Nerves Center, since March 10, 2008, the invoice number (*) has been printed on the bank account name column.

※The invoice number is to be assigned an arbitrary number (within 6 digits, only number) to the invoice so that you can check the details of the transfer. When submitting an invoice, please fill in the invoice number.

The name of the transferee will be "Yokohamashi T XXXXXX".

※○○○○○○The portion is the invoice number.

(Example) In the case of invoice number 123456: “Yokohamashi T123456”

Payments made by invoices without the invoice number will be printed on the passbook as follows.

• Payment of a municipal hospital ... ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ ・ “Yokohamasimin Hiyowi”
• Stroke / Payment of Neurospinal Center ... "Nosotsuchiyushinke"

• ※On January 1, 2015, Yokohama Stroke & Brain Center was renamed the Stroke and Neurospinal Center.
