Consultation over the phone
Consultation hours are from 8:45 to 17:15 (excluding 12:00 to 13:00) from Monday to Friday, excluding holidays and New Year's holidays.
Consultation by fax, e-mail, or letter
In the case of consultation by fax, e-mail, or letter, the Welfare Coordination Committee Secretariat may ask you to inform us of the situation in detail by telephone or other means.
Please note that due to holidays, the contact from the secretariat may be delayed.
If you are contacted by fax, e-mail, or letter, be sure to fill in the following:
1. Your name (furigana)
2. Phone number (I would like to contact you during the day.)
3. Service name, business name, etc. for complaints and consultation
(Example) Type of allowance, type of facility, name of service used, etc.
4. Details of complaints and consultations (please specify)
