Issuance of notebooks

• In Yokohama City, a notebook for people with intellectual disabilities (care notebook) is called a notebook of love.
• Psychological judgment is made, and judgment is made according to the degree of disability, such as A1, A2, B1, and B2.
• Depending on the degree of disability, various services are available.
• It will take about 2 months to issue a notebook.

• This is a notebook for those who have the following disabilities. Visual impairment, hearing and balance impairment, speech and language impairment or masticatory impairment, physical disability, internal dysfunction
• Depending on the degree of disability, the grade of the notebook is grade 1 (severe) to grade 6 (mild).
• Various services are available depending on the type and grade of disability.
• It takes about one and a half to two months to determine the issuance of the notebook.

※From January 2016, a personal number (My Number) is required for the certificate of the physically disabled.

Since the format differs for each target person, please download (PDF file) from the following window or from the Health and Social Welfare Bureau homepage to obtain the prescribed medical certificate form. In addition, please receive diagnosis of Article 15 designated doctor prescribed by the Welfare Law for the Physically Disabled, and request the creation of a medical certificate.
