Target person

Vision, auditory, balance function, voice, language function or masticatory function, limb (upper limb / lower limb / trunk / brain Hara motor dysfunction), heart function, kidney function, respiratory function, bladder or rectum function, small intestine function, immune function Those who have permanent impairment in liver function

Disability class

Depending on the degree of disability, you will be certified from grade 1 to 7. (However, if there is only one 7th grade disability is not eligible for the notebook.)
The content of the support may vary depending on the grade.

Flow to obtain a personal organizer

1.Prepared medical certificate

A description by a designated doctor under the Welfare Law for the Physically Handicapped is required.
Please consult with family doctor in advance to obtain a personal organizer.

2.Application

What you need

A personal number (My Number) is required for the certificate of the physically disabled.

When the person applies.
  • Face photo
  • Medical certificate of the physically handicapped (diagnosisted by a designated doctor)
  • Identity verification documents
When proxy/agent applies
  • Face photo
  • Medical certificate of the physically handicapped (diagnosisted by a designated doctor)
  • Identification documents of the person who came to the counter
  • Documents confirming the right of proxy (such as power of attorney)

[Precautions for Photos]
 1. Photographs of upper body taken recently (within one year). 4cm long x 3cm wide.
 2. You must have no hat. (Except when there is a religious or medical reason.)
 3. Wearing sunglasses and masks are not allowed.
 4. Photos with as little background as possible, such as ID photos. It doesn't show anyone else.

[About Braille Seal]
For those who are visually impaired who have a notebook from Motoichi, we will give you a sticker with the notebook number in Braille. When applying, please contact us at the counter.

3.Grants

Period

After application, it takes about one and a half to two months.

About the style of notebook

Yokohama City introduced the card style of the Handbook for Persons with Disabilities in June 2021. When applying, you can choose either paper form or card form.
If you already have a notebook and do not wish to issue a card style, you can use your current paper-style notebook as it is.
If you have a notebook from Motoichi and wish to change from paper style to card style, please click here.

About mail application

 If you already have a certificate of the physically disabled in Yokohama City (if you moved in from another city, you have completed the transfer procedure for the notebook), mail only for applications for reissuance of "when lost notebook" and "when notebook is damaged" We accept by mail.

Precautions for mailing applications

 ・A letter stating that the notebook has been completed will be sent to the person (target person) Address.
 ・The completed notebook is issued at the Health and Welfare Center of each ward office.
