A description by a designated doctor under the Welfare Law for the Physically Disabled is required.

A personal number (My Number) is required for the certificate of the physically disabled.

In Yokohama City, we introduced the card style of the Handbook for Persons with Disabilities in June 2021. At the time of application, you can choose either paper style or card style.
If you already have a notebook and do not wish to issue a card style, you can use your current paper-style notebook as it is.

Regarding the certificate of the physically handicapped for those who have entered (entered) facilities for the disabled or facilities for the elderly, if the municipalities where the facilities are located are uniformly implemented as the implementing entity, the municipalities with many facilities, etc. The burden of office work is concentrated. A mechanism established to correct such imbalances is the "exception of residence".
Those who have entered (entered) a facility subject to the special place of residence will not have to apply for a certificate of the physically disabled at the municipality where the facility is located, but will go to the municipality where they originally lived.
However, for applications that require a certificate of the physically disabled and a statement of opinion (new application, grade change application, additional disability application, recertification application), procedures will be taken at the municipalities where the entrance facility is located.
