Procedures and applications by mail

Notifications and applications for Insurance and Pension Division (National Health Insurance, The Long-term Care Insurance, Medical System for Elderly Senior Citizens, medical expenses Subsidy Program) are also handled by mail. Please check the explanation below and the link before sending.
◎Notes
・Please fill out the documents with an unerasable writing instrument such as a ballpoint pen. You can't use pencils or eraseable ballpoint pens.
・For private seal, please use private seal, which uses vermilion. You can't use the stamp.
・Please pay the mailing fee (except if you have a reply envelope) 。
・Please note that if the documents are incomplete, we will return the complete set of documents.

National Health Insurance

Information on procedures and forms for withdrawal from National Health Insurance through the participation of other employee health insurance.

Nursing care insurance

Reports related to The Long-term Care Insurance, explanations and forms related to application are posted.
If you wish to submit a report or application by mail, please contact the ward office in advance.

About change of destination of care insurance card, payment notice (when mail does not arrive in Address on resident registration such as entrance to facility), we mail application form, so please ask ward office.

You cannot apply by mail. Please come to the ward office window.
