 A person who intends to submit a proposal must be confirmed that all of the following conditions are met and that they are eligible to submit a proposal.

(1)Being not a person listed in Article 3, Paragraph 1 of the Yokohama City Contract Regulations (March 1964, Yokohama City Regulations No. 59) and being a person who has the qualifications stipulated by the provisions of Paragraph 2 of the same Article.

(2)In FY31/32 Yokohama City General Competitive Bidding Qualified Person List (Related to Goods / Consignment, etc.) "A: Market / Public Opinion Survey" and "B: Consulting (Construction Consultants, etc.) ) ".

(3)A person who has not received the suspension of nomination based on the Yokohama City Nomination Suspension Measures Guidelines on any day between the submission deadline of the registration form and the specific date of the contract candidate.

(4)A person who has a track record of conducting surveys and examinations on the construction or rebuilding of a financial accounting system.

Notification of participation qualification confirmation result and request for submission

[Notice Concerning Participation]
 Among those who have submitted the application form for participation, those who have been granted the proposal qualification and those who have not been accepted will be notified of that fact and the reason thereof by the proposal qualification confirmation result notification.
 Notifications will be sent via e-mail.

Notification date: April 26, 2019 (Friday)

[Submission of Proposal]
 We will issue a proposal-related document submission request form to those who have been approved for proposal qualification, along with a proposal qualification confirmation result notice. Please submit the proposal based on the guidelines for preparing the proposal.
 Delivery will be made by e-mail.

8 copies (7th, 1 copy for copying)

  • Plug the form 6 to 8.
  • For copying, do not staple the clip without stapler.
  • Please submit one copy of the document (contract, copy of specifications, etc.) that shows the results of Form 5 and Proposal Creation Guidelines 7 (1), without putting them in the staple staplings of Forms 6 to 8 .

4.Others

  • We will not accept documents other than the prescribed form.
  • In Forms 6-8, do not include any company name or other information that can be identified by a glance.
