About submission of notification by mail

 When submitting a notification by mail, the mailing address is as follows.

 If you wish to return a copy, please enclose a notification of the number of copies (*) with the number of copies to be submitted and an envelope (with a stamp and the destination address).
 ※ Storage and Disposal Status, etc. registration form, end of disposal, and end of disposal registration form submitted 2 copies (3 copies if a copy is required).
   1 copy of other reports (2 copies if a copy is required)

Storage and Disposal Status, etc. registration form

(1) Target companies

 Store PCB waste or owning products using high-concentration PCB
  ※If you keep it continuously, you need to submit a notification every year.
  ※Even if all disposals are completed during the previous year, it is necessary to submit until the notification for this year.

(2) Details of notification

 Please notify the storage and disposal status of PCB waste and products using PCB in the previous fiscal year between April 1 and June 30.
  ※When disposed of, please attach a copy of the D or E form of the manifest (industrial waste management slip).

New Occurrence Report

(1) Target companies

 Businesses that have newly generated PCB waste
  ※Those who fall under any of the following reasons are eligible.
   ・When a new PCB waste is discovered
   ・When products using PCB are removed from the electric circuit
   ・When it is determined that it is PCB waste based on the results of a PCB content survey, etc.
   ・When samples or pollutants are generated due to sampling or oil extraction work.

(2) Details of notification

 Please notify us immediately after PCB waste is generated.

Change of transportation plan and storage location registration form

(1) Target companies

 (a) Storage companies (before transporting) that transports PCB waste to change the business sites that store PCB waste
 (b) Businesses that store PCB waste or change the location of products using high-concentration PCBs (after transportation)
  ※It is applicable in any case of "transportation in the city", "transportation from outside the city to the city", "transportation from the city to the suburbs".

(2) Details of notification

 (a) Before transporting
  Please submit a notification before transporting PCB waste.

 (b) After transporting
  Please notify us within 10 days from the date you change the storage location of PCB waste.

Change in storage and usage registration form

(1) Target companies

 Storage and Disposal Status, etc. Businesses whose contents of notification by registration form (Form No. 1) have been changed
  ※Those who fall under any of the following reasons are eligible.
   ・When it is determined from the results of a PCB content survey, etc. that it is not a PCB waste or a product that uses PCB.
   ・When it is found that the quantity and type of PCB waste in storage or products using PCB are different.

(2) Details of notification

 Please notify us within 30 days from the date on which the changes are found.

End of disposal and end of disposal registration form

(1) Target companies

 (a) When all high-concentration PCB waste stored is disposed of
 (b) When all low-concentration PCB waste stored is disposed of
 (c) When all products that use high-concentration PCBs are disposed of

(2) Details of notification

 (a) (b) When disposed of
  Please notify us within 20 days from the date of entrusting the disposal (the date of concluding the disposal consignment contract).
 (c) When disposed of
  Please notify us within 20 days from the date of disposal (the day you stopped using it).
