About submission of notification by mail

 When submitting a report by mail, the mailing address is as follows.

 If you wish to return a copy, please enclose a notification of the number of copies to be submitted plus the copy (*) and an envelope (with a stamp and the destination is stated).
 ※ Storage and Disposal Status, etc. registration form, End of Disposal and Disposal of registration form (3 copies if a copy is required)
   One copy of the other notification (two copies in total if a copy is required)

Storage and Disposal Status, etc. registration form

(1) Target businesses

 Owners of PCB waste or own products that use high-concentration PCB
  ※If you keep it continuously, you need to submit a notification every year.
  ※Even if all disposals are completed during the previous fiscal year, it is necessary to submit it until the notification for this year.

(2) Details of notification

 Please notify us of the storage and disposal status of PCB waste and products using PCBs in the previous fiscal year between April 1 and June 30.
  ※If you dispose of it, please attach a copy of the D or E form of the manifest (industrial waste management slip).

New Occurrence Report

(1) Target businesses

 Businesses that have newly generated PCB waste
  ※Those who fall under the following circumstances are eligible.
   ・When a new PCB waste is discovered
   ・When a product using PCB is removed from an electric circuit
   ・When it is determined to be PCB waste based on the results of a PCB content survey, etc.
   ・When samples or contaminants are generated due to sampling or oil extraction work

(2) Details of notification

 Please notify us as soon as the PCB waste is generated.

Change inregistration form

(1) Target businesses

 (a) Storage companies that transport to change the business site where PCB waste is stored (before transport)
 (b) Businesses that have stored PCB waste or changed the location of products using high-concentration PCBs (after transportation)
  ※All cases of "transportation in the city", "transportation from outside the city to the city", and "transportation from the city to the suburbs" are eligible.

(2) Details of notification

 (a) Before transporting
  Please notify us before carrying PCB waste.
   ※For the attachments, please refer to the documentation.

 (b) After transporting
  Please notify us within 10 days from the date you change the storage location of PCB waste.

Changes in storage and usage status registration form

(1) Target businesses

 Businesses whose contents of notification by registration form (Form No. 1), such as storage and disposal status
  ※Those who fall under the following circumstances are eligible.
   ・When it is determined that the product is not PCB waste or PCB-based product based on the results of a PCB content survey, etc.
   ・When it is found that the quantity and type of PCB waste in storage or products using PCB are different

(2) Details of notification

 Please notify us within 30 days from the date on which the change is found.
  ※For the attachments, please refer to the documentation.

End of disposal and disposal registration form

(1) Target businesses

 (a) When all high-concentration PCB waste is disposed of
 (b) When all the low-concentration PCB waste is disposed of
 (c) When all products using high-concentration PCBs used are disposed of

(2) Details of notification

 (a) (b) When disposed of
  Please notify us within 20 days from the date of entrusting the disposal (date of the disposal consignment contract).
 (c) When disposed of
  Please notify us within 20 days from the day when disposal was completed (the day when use was stopped).
  
    End disposal of polychlorinated biphenyl waste or termination of products using high-concentration polychlorinated biphenyl registration form [Form No. 4]
