[Note] About postage pertaining to the reply envelope of the addition notice submitted after September 1, 2024

  • Since the postage will be changed from October 1, 2024, the postage fee to be affixed to the reply envelope pertaining to the notification of addition will be changed for the "additional notification after September 1, 2024". Please attach a stamp for "new fee".

 ※Please check the post office website (outside site) for details on the postage change.

1.When notification is required

  • When we are going to receive application of addition that requires prior notification
  • When it no longer meets the requirements for addition
  • When there is a change in the content of the notification
  • When attempting to apply for designation
  • When a notification item is added or changed due to a law revision, etc.

2.Documents to be submitted

Office scale inspections are updated every year. For the latest version, go to ↓ “7. Office scale inspection”.

3.Date of submission

・Please submit by the 15th of the previous month of the month in which the addition is calculated.
・Also, if you no longer meet the requirements for addition, please submit it promptly regardless of the above date and time.

4.Submission method

Please refer to the separate sheet "★ required documents List" and submit required documents to Nursing Care Business Guidance Division by mail (bring) or electronic application notification system (Ministry of Health, Labor and Welfare).

7.Office scale inspection

★Fiscal 2024 Remuneration Revision Information
With the revision of the remuneration in FY2024, the scale classification has been integrated from three types: "Normal scale", "Large scale", and "Large scale II" to two types, "Normal scale" and "Large scale", and establishments that meet the requirements Is "large scale (exception)" and the basic remuneration is the same as normal scale.
When you make scale division "large-scale (exception)", please submit addition notice by last month 15.

[Important]
For establishments with a scale category of "Large-scale I" or "Large-scale II" in 2024, it is necessary to change the scale category to either "Large-scale" or "Large-scale (exception)".
Check the submitted documents in the notification of addition, and submit the notification of addition by May 15, 2024 (Wednesday).

Inspections are required at the time of new designation applications and every year (to determine the scale of the next fiscal year). When scale change is necessary, it is necessary to report to Motoichi by March 15. Please refer to “5. Submission address” for the submission address. It is style of establishment scale check book. Use this for existing offices (the previous year's results were more than June). At the same time, please refer to the description example (PDF: 389KB).
※If you provide a short-term “home visit service” or “safety confirmation service by telephone” based on “Temporary handling of personnel standards for nursing care service establishments related to COVID-19 infection (2nd, 4th and 6th report)” and calculate the nursing care fee, calculate the calculated category (“2 hours or more and less than 3 hours” for tsusho rehabilitation, “1 hour or more and less than 2 hours”).
