1 Home service [Notification of Change, Abolition, Suspension, Reopening]

1.When notification is required

1) When change occurs, please submit change registration form to Motoichi without delay.
・Location of business sites
・Changes in managerial offices
・Phone number and fax numbers of offices and corporations
※Please confirm item that you need to report to Motoichi in "Notification of change and required documents" of "2. Submission Documents". (The reasons for need to submit a change notification vary depending on the service.)
※If you submit a new notification of addition due to this change, please be prepared to submit it at the same time because the notification of change will be a requirement for addition calculation. It is possible to submit separately by each submission deadline, but please note that if the notification of change is an additional calculation requirement, it will be reviewed after both can be confirmed.
(For example, when the number of employees in care manager is calculated by adding one person.)

2) required documents's "Registration Information Service" can be used for "Registration Information Service".
This is a paid service that allows you to check the registration information held by the registry office on the computer screen using the Internet.
Instead of submitting an entry certificate (original) to Yokohama City, Yokohama City can check the registration information on the system by notifying the reference number issued by the service, so there is no need to mail the original.

[Change Notification] If there are multiple reasons for changing the same date of move, attached documents submissions are not required.

★About setting, change and abolition of branch office (satellite) of designated home-visit nursing establishment (except deemed designation), report is necessary as notification of change pertaining to main business establishment. ★When cooperation of regular patrol, occasional correspondence type home visit care nursing establishment increases (reduction), report is necessary in Yokohama-shi electronic application, report system (outside site).

Care Staff Treatment Improvement Results Report

If you calculate the care staff treatment improvement addition, please submit it by the last day of the month following the month when you received the final nursing care fee.
About styles of results report, please confirm "about care staff treatment improvement addition".

★Example: Scheduled to be abolished on August 31

3.Confirmation of submission method-There are two methods: "Submission of documents" and "Electronic application"

※In order to use the "Electronic Application Notification System (Ministry of Health, Labour and Welfare)" requires registration of "G Biz ID".
