1 Home service [Change, Abolition, Suspension, Reopening Notification]

[Note] Regarding postage pertaining to the reply envelope for notification of change submitted after September 1, 2024
- Since the postage will be changed from October 1, 2024, the postage fee to be affixed to the reply envelope pertaining to the notification of change will be changed for "Notification of change after September 1, 2024". Please attach a stamp for "new fee".

1.When notification is required
1) When change occurs, please submit change registration form to Motoichi without delay.
・Location of Business Sites
・Change of manager of office
・Phone number, fax numbers, etc. of offices and corporations
※Before submitting the notification, please check the items that need to be notified to Motoichi in “Notification of Change and required documents” in “2. Documents to be submitted”. (The reason for submitting a notification of change differs depending on the service.)
※If you submit a new notification of addition, etc. due to this change, the notification of change will be a requirement for addition calculation, so please prepare so that you can submit it at the same time. It is possible to submit separately by the deadline for each submission, but please note that if the notification of change becomes an additional calculation requirement, both will be reviewed after confirmation.
(For example, when calculating the addition of a specific business establishment due to an additional one at care manager)
2) You can use the registration information service for the “Certificate of Registration (original)” in required documents.
This is a paid service that allows you to check the registration information held by the registry office on the screen of a PC using the Internet.
Instead of submitting a certificate of entry (original) to Yokohama City, by notifying the reference number issued by the service, Yokohama City can check the registration information on the system, eliminating the need to mail the original etc. .
