The SOS Network for Tsuzuki Ward Elderly People with Dementia is a mechanism for the ward office and related organizations to work together to find out the person as soon as possible when elderly people with dementia go out and go missing. .

Pre-registration
Information (physical characteristics, etc.) of people with dementia who may be missing can be registered in advance at the ward office or Local Elderly Care Management Center.
About those who can use it
- Those who live in Tsuzuki Ward and are over 40 years old and may be missing due to dementia. (Registration of those who live in dementia group homes is also possible.)
What is required for registration
- Photos (Those that show the characteristics of the recent person, two types of face photo and whole body photo, not returnable)
- Registration application form (you can also hand it over at the Tsuzuki Ward Office or Regional Comprehensive Support Center)

About the registered information
Pre-registration information will be stored and managed at Elderly and Disabled Support Division Tsuzuki Ward Office, Regional Comprehensive Center and Tsuzuki police department.
If you wish, we will provide registration information to Kanagawa Prefectural Police.
Providing pre-registration information to the Kanagawa Prefectural Police leads to a quick investigation by the prefectural police and early resolution can be achieved.

When it goes missing
- Please call police department Tsuzuki first.
- Next, please contact the Tsuzuki Ward Office Elderly and Disabled Support Division (or Regional Comprehensive Support Center).
- The ward office (or Local Elderly Care Management Center) that has been contacted will request cooperation from cooperating organizations (public institutions, transportation, etc.) for cooperation. Related organizations (organizations that share pre-registration information and cooperating organizations) will work together to plan early detection of missing people.
If the person is found
- The relevant organization will contact police department Tsuzuki.
- After police department has protected it, he will return to his family.
- Family members should report their return to the Tsuzuki Ward Office Elderly and Disabled Support Division (or Regional Comprehensive Support Center) that they have returned.
