About Nishi-Public Works Office road width certification application procedure

For inquiries regarding the application, please contact Nishi Public Works Office Administration Section (Tel: 045-242-1313).

• It takes about one week from the reception to issue the certificate.
• When you receive the certificate, you will need certificate stamp worth 300 yen per case, so please purchase the certificate stamp in advance at the ward office before visiting the office.
• If you receive a road width certification request, please submit two copies of the application form.

• If the applicant is an individual, please stamp the individual's seal, and if it is a corporation, the representative's seal or company's seal.
• Please fill in the lot number of the place where you want to receive the road width certificate in the column of application form "1".
• Please fill in the reason for the need for certification in the column of “2” for application form.
• Please do not fill in the column of application form "3" here.

• Please fill in the form by hand.
• Please indicate the entrance to the garage.
