Vaccinations

　　If you receive routine immunization at a medical institution outside the city, you will need to apply in advance at the ward office, Health and Welfare Division Health Promotion Section. Based on the Vaccination Law, we will issue a “Vaccination Implementation Request Form”, which is a document that clarifies that The Mayor of Yokohama is responsible for its implementation.
　
・ If you have a resident registration in Minami-ku, Yokohama City as of the day of vaccination and receive routine immunization at a medical institution outside the city, if you fall under the following (1) or (2), apply online from the link below. Is possible.
　　（１） Homecoming births outside the city (including births of younger brothers and sisters)

　　　　　・User registration of the Yokohama City Electronic Application System is required in advance.
　　　　　・We will call you from Health Promotion Section Minami Ward Office Health and Welfare Division within three business days after your application.
　　　　　・It takes about one week to 10 days from application to issuance of the implementation request form.
　
◎About inoculation costs
　If you are vaccinated outside the city, you will be responsible for all vaccination costs.
　However, in the case of (1) or (2) above, you can receive a refund (reimbursement payment) of the vaccination fee by receiving a “Vaccination Implementation Request Form” in advance. About refund method, we send information together with "vaccination conduct request book".
