About notification of infectious disease

To medical institutions
Depending on the type of infectious disease, a notification to Health and Welfare Center is required.
Please see the homepage of Yokohama City Inst. of Health for details such as notification standard, outbreak notification style.
For social welfare facilities, etc., The Long-term Care Insurance operators and nursery schools
If a patient with symptoms such as diarrhea or vomiting, or if a patient is diagnosed with influenza or symptoms of flu, and tend to spread, please contact the South Health and Welfare Center immediately.
If the following applies, you will need to contact South Health and Welfare Center.
1: When two or more fatal or serious patients occur within a week due to the same infectious disease or food poisoning.
2: When there are 10 or more patients with the same infectious disease or food poisoning or suspected of them or more or half of all users
Even if it does not fall under 3: 1 and 2, it is suspected that an outbreak of infectious disease etc. exceeding the normal outbreak trend, especially when the facility manager deems it necessary to report

When class, grade, or facility closure due to influenza
■Facilities subject to reporting
Kindergarten, elementary school, junior high school, high school, etc. (secondary school, special needs schools, vocational schools and various schools)
※Junior colleges and universities are not eligible.
■Reasons for Coverage
When class is closed, school year closure, or facility closure due to influenza or influenza-like illness
■Reporting Procedure
Target facilities are to Minami Health and Welfare Center Health and Welfare Division Health Promotion Section (Tel: 045-341-1185, fax: 045-341-1185, fax: 045-341-1189) by noon on the day when the class was closed according to "01 Form 1 Influenza-like Disease Occurrence Report" (Excel: 198KB) Please report.
