Notification of infectious diseases, etc.

To medical institutions
Depending on the type of infectious disease, it is necessary to notify the Health and Welfare Center.
Please see the homepage of Yokohama City Inst. of Health for details such as notification standards and outbreak notification styles.
For those involved in social welfare facilities, The Long-term Care Insurance operators and nursery schools
If there are patients with symptoms such as diarrhea or vomiting, or if there are cases where flu diagnosis or flu symptoms appear and tend to spread, contact Minami Health and Welfare Center immediately.
If the following applies, you need to contact and notify the South Health and Welfare Center.
1: When two or more fatal or serious patients occur due to the same infectious disease or food poisoning or suspected to be caused by them in one week.
2: When more than 10 patients with the same infectious disease or food poisoning or suspected persons occur or more than half of all users
3: Even if it does not fall under 1 and 2, outbreaks of infectious diseases that exceed normal outbreak trends are suspected, especially when the facility manager deems it necessary to report.

When closing classes, grades, and facilities due to influenza
■Facilities subject to reporting
Kindergarten, elementary school, junior high school, high school, etc. (secondary school, special support school technical college, vocational school and various schools)
※Junior colleges and universities are not eligible.
■Reason for Applicable
When class closure, grade closure, or facility closure due to influenza or influenza-like illness
■Reporting procedures
Please report to Health Promotion Section Health and Welfare Division, Minami Health and Welfare Center (Tel: 045-341-1185, Fax: 045-341-1189) by noon on the day of the decision to close the class in accordance with “Form 1 Influenza-like Disease Report” (Excel: 198KB).
