Survey target…4,500 Midori Ward residents over 20 years old (including 120 foreign nationals, randomly selected from the Basic Resident Register)

How to answer…If you receive it, please answer it in a sealed letter or the Internet. (In the case of an answer via the Internet, please answer from the URL or two-dimensional code described in the questionnaire.)
