Please make a reservation from the following electronic application system.
Applications can be made from 30 days before the reservation date to 3 business days before the reservation date. Cancellations must be made up to 3 business days before the scheduled date of the next agency (reservation date).

①Enter the top screen at the following URL and register the user ID and password on the system.

②After registering your user ID and password, proceed to the reservation screen according to the following procedure and make a reservation.

　→　Click "family register Tax/Insurance" from "Category" in "Search by specifying conditions".　
　→　Please make a reservation from "Reservation for notification and consultation of" marriage "" marriage "" (Konan Ward Office)".

◯"birth registration form" is within 14 days including the date on which the child was born, and within 3 months including the date of birth (if the 14th day is a ward office closed day, the next open day).
◯Please note that the report may not be completed on the spot due to lack of notification contents or documents when you come to the agency.
◯This reservation system is carried out by the Digital Ward Office model. We do not carry out in other wards.
◯Please note that you cannot make a reservation by phone.
◯"birth registration form" or "marriage registration form" consultation is limited to those who submit a notification in Konan Ward.
◯Notifications and consultations are also accepted without reservation. If the notification / consultation other than the above, or if the above reservation slot is filled, please come directly to the counter.
◯Please refrain from canceling. In the unlikely event that cancellation due to an accident or other reasons, please contact us by phone at least one business day before the reservation date.

③Notifications and consultations will be conducted until the end of the meeting.
