Konan Ward Original Birth Notification Proof (started on February 1, 2024)
We will begin issuing “Konan Ward Original Birth Notification Proof” to those who submit birth registration form to the Konan Ward Office. The birth notification proof is a proof in the Maternal and Child Health Handbook that you have reported birth registration form.

Targets
Those who submitted birth registration form after February 1, 2024
Those who submitted birth registration form before February 1, 2024 and who have already received a certificate of birth notification are not eligible.
Method of receipt
When your child is born, please submit birth registration form to window 20 on the second floor of Family Registry Division. (within 14 days) At that time, you will need to bring your Maternal and Child Health Handbook and certify your birth notification. If it is difficult to bring it on the day, please bring it with you at a later date.
