Proof of birth notification completed in Konan Ward (issued on February 1, 2024)
"Konan Ward Original Birth Notification Certificate" will be issued to those who submit birth registration form to the Konan Ward Office. The birth notification certificate is a proof in the Maternal and Child Health Handbook that you have reported birth registration form.

You can choose one of the above five types. In addition, you can also choose a plain certificate without a design. Also, if you think about which design to use in advance, the reception will be smoother.

Targets
Those who submitted birth registration form on or after February 1, 2024
Those who submitted birth registration form before February 1, 2024 and have already been issued a birth notification certificate are not eligible.
Receipt method
When your child is born, please submit birth registration form to the 20th window on the second floor of Family Registry Division. (Within 14 days) At that time, please bring your Maternal and Child Health Handbook and certify your birth notification. If it is difficult to bring it on the day, please bring it at a later date.
