As one of the celebrations of life events for residents of the city, we created a certificate of acceptance of marriage registration form, which was originally designed by Konan Ward. How about celebrating the submission of marriage registration form in Konan Ward?

3 types of ward original designs
①Sunflower scenery I imagined sunflower scenery in Konan Ward.
②Sijukara The motif is the bird of the ward, and the image is inspired by a stained glass.
③83 Taro We decorated the flowers of the ward with 83 Taro with a peaceful impression.
Conventional city common design (certificate type)
Three types of designs were created based on the motifs of the flower sunflowers of the ward, the bird of the ward, and the 83 Taro of the Konan sunflower 83 campaign promotion character.

How to apply
Please fill in the desired design name and other required items on the invoice for family register Certification, etc., and request it at the 20th floor of Konan Ward Office.
※Invoices such as family register certification are provided at the ward office and at the entry table.
Receiving method and number of days required for receipt
If you apply by the 20th of the month following the date of submission by marriage registration form (the date of receipt), it will take about one week to complete the application. We will contact you as soon as it is completed, so please come to the agency after that.
※If you apply for more than 20 days of the following month, it may take up to one month to create, so please consult us.
※Please prepare your identity verification materials at the time of receipt.
Size
All sizes are B4 versions (3 types of ward original designs and city common design (certificate type)). A4 version (350 yen) is issued only in the city common style.
