Konan Ward Original Certificate of Acceptance of marriage registration form

The design was created with three motifs based on the flower sunflower of the ward, the bird tit of the ward, and Taro 83 Taro 83, a character of Konan sunflower 83 movement promotion character.
We issue marriage registration form acceptance certificate (certificate type) common to the 18 wards of the city that has been issued before.

Those who have submitted marriage registration form to Konan Ward (past notifications are also applicable.)

Please write the name of your design and other required items on the invoice for family register Certification, etc., and request it at the 20th window on the 2nd floor of the Konan Ward Office.
※Invoices such as family register certification are provided at the ward office window and entry stand.

If you apply by the 20th of the month following the submission date of marriage registration form (the date of receipt), it will take about one week to complete the application. We will contact you as soon as it is completed, so please come to the agency.
※If you apply for more than 20 days of the following month, it may take up to one month to create, so please consult us.
※Please prepare your identity verification document when you receive it.

All sizes are B4 versions (3 types of ward original designs and city common design (certificate of diploma type)). A4 version (350 yen) is issued only in the city common style.
