About infant health examination
Infant health checkups are an opportunity to check the growth of your child at the milestone of growth and support child care, and it is important to have them take them at a better time.
3-year-old Health Checkups
We will notify you individually in advance.
Please fill out the enclosed questionnaire and bring it with you.
Requests for having a medical checkup with peace of mind
（１） If you find it inconvenient on the designated medical examination day, please come to the next medical examination day.
（２） From March 13, 2023 (Monday), individual judgments are the basis for wearing masks.
（３） If you have any physical condition, please refrain from consulting.
（４） Thank you for your cooperation in taking out used diapers. Please bring your filth bag.

Urine testing, measurement, medical examination, dental checkup, childcare consultation (discipline, diet, teeth, development), etc.
※I have an urinalysis. Please take the urine of the day in a container distributed in advance.

Maternal and Child Health Handbook (Main Volume), mailed medical questionnaire, writing utensils, urine, diaper
