About infant health examination
Infant health checkups are an opportunity to check the growth of your child at the milestone of growth and support child care, and it is important to have them take them at a better time.
4-month-old child Health Checkups
We will notify you individually in advance.
Please fill out the questionnaire enclosed with the individual notice and bring it with you.
Requests for having a medical checkup with peace of mind
（１） If you find it inconvenient on the designated medical examination day, please come to the next medical examination day.
（２） From Monday, March 13, 2023, personal judgment was the basis for wearing masks.
（３） If you have any physical condition, please refrain from consulting.
（４） Thank you for your cooperation in taking out used diapers. Please bring your filth bag.

Measurement, consultation, childcare consultation (baby food, teeth, motherhood, development) and others
◆Your belongings of the day
Maternal and Child Health Handbook (Main Volume), mailed medical questionnaire, writing utensils, bath towels, diapers

From 0:30 p.m. to 1:30 p.m.

From around 0:30 p.m.
