You must bring your ID card (for those aged 65 and over), employee health insurance insured card (for those aged 40 to 64), a memo that shows the name of your medical institution, location, and family doctor name, patient's card Shinryo-ken, and private seal (not required when youThe Long-term Care Insurance is present).
