What is required to issue a certificate to receive the service?

Please prepare the following documents depending on the type of certificate.
- Disability certificate, proof for NHK reception fee reduction → Handbook for the Physically Handicapped or Handbook for Love
- Certificate of Storage → Notice of Child Raising Allowance Decision
- Certificate of Negaki (certificate applicable to standard) → Long-term care insurance card
- Recipient Certificate → Certificates of various benefits
※ Please contact us in advance for certificates for NHK reduction and exemption, as there are applicable requirements.
