What do I need to issue a certificate to receive the service?

Please prepare the following documents depending on the type of certificate.
- Disability certificate, certificate for NHK reception fee reduction → Handbook for the Physically Handicapped or Love Book
- Certificate storage certificate → Child Raising Allowance Decision Notice
- Neglecting certificate (certificate applicable to standard) → Long-term care insurance card
- Recipient certificate → Certificates of various benefits
※ Please contact us in advance as there are requirements for NHK reduction of taxes and negaki certificate.
