About community medicine concept

Based on the provisions of the Medical Law enforced in April 2015, the Kanagawa Prefectural Community Health Plan, which shows the direction of long-term efforts to build a medical provision system that should be in 2025, was formulated. Was.

（１） Purpose of the formulation
Toward 2025, when baby boomers will be 75 years of age or older, secure bed functions that will be insufficient in the future, build a cooperative system, and promote a comprehensive community care system.
It shows the direction of the efforts with the aim of enhancing home healthcare for the purpose of securing and training human resources who support them.

（２） What we aim for the future
"Kanagawa where everyone can live energetically and lively and receive high-quality medical care and nursing care in a familiar area when needed."

（３） Three Initiatives in the Community Health Initiative
For realization of "target person" to show in community medicine design, we work on matter to advocate next in cooperation with affiliate or citizen.

(A) Securing bed functions that will be insufficient in the future and building a cooperative system
(B) Enhancement of home medical care for the promotion of a comprehensive community care system
C. Securing and training medical staff to support the future medical provision system
