1. Kanagawa Prefecture Specific Disease medical care certificate Grant Application Form
2. Clinical survey personal card for the application disease (within 3 months from the date of the doctor's statement)
3. Copy of health insurance card
4. Copy of resident certificate (within three months from the date of issue with the relationship of all households)
5. If you are a member of employee insurance (Kempo Association, Health Insurance Association, Mutual Aid Association, etc. employee health insurance where you work), you must agree to provide information on high medical costs's income category from the insurer.
6. If you are a member of a National Health Insurance union other than Kanagawa Prefecture-approved (Kanagawa Prefecture Doctor, Dentists, Food Sanitation, Pharmacists, Construction Union National Health Insurance Association), municipal inhabitants of union members and all insured persons Tax (non) tax certificate (copy of municipal tax tax payment letter of advice etc. is not allowed)
