• Documents required for new application

3. Copy of health insurance card
4. Copy of resident certificate (within three months from the date of issue with the relationship of all households)
5. Those who join Employee Insurance (employee health insurance, Health Insurance Association, Mutual Aid Association, etc.) are required to provide consent form (approval from the insurer to provide information on high medical costs's income classification).
6. Those who are members of the National Health Insurance Union other than Kanagawa Prefecture approved (Kanagawa Prefectural Doctors, Dentist, Food Sanitation, Pharmacists, Construction Industry, Construction Union National Health and Health Association) Tax (No copy of municipal inhabitant tax payment letter of advice etc. is not allowed)
