Article 15 of the Handbook for the Physically Disabled What is a Designated Doctor?
Designated doctors based on the provisions of Article 15 of the Welfare Law for the Physically Disabled (hereinafter referred to as "Article 15 Designated Doctors") are doctors who create "Medical Certificates and Statement of Opinion" necessary for applying for a certificate of the physically disabled. That is.
Article 15 The designation of a designated doctor is made after the prefectural governor (including the designated city mayor and the core mayor) listens to the opinion of the local social welfare council according to the location of the medical institution.
In Yokohama-shi, we hold Yokohama-shi social welfare council obstacle degree examination sectional meeting four times a year (March, June, September, December), and perform designation after hearing the opinion.

Article 15 Procedures for Designation of Designated Doctors
Doctor who hopes for designation attaches necessary documents after mentioning in the next application, and please submit to Yokohama-shi person with a disability rehabilitation consultation office by deadline.
In addition, even if you have been designated outside Kanagawa Prefecture, if you are transferred to a medical institution in Yokohama City, please note that you will need to apply for a new designation application.

The deadline for submitting application documents is until the 10th of the previous month of the examination month (March, June, September, December).
Article 15 Transfer, decline, etc. of designated doctors

When a medical institution is transferred
If the designated doctor changes the medical institution to which he / she belongs, a "Notification of Change" is required.
When transferring a medical institution in Kanagawa Prefecture
Please submit "transfer notice" to each local government (prefecture, designated city, core city) before transference and local government (prefecture, designated city, core city) after transference.
When transferring to a medical institution outside Kanagawa Prefecture
Please submit "notice of transfer" to Yokohama-shi, and please apply for designation again to local government after transference.

※If you are transferred from a medical institution outside Kanagawa Prefecture to a medical institution in Yokohama City, a new designation in Yokohama City is required.
When concurrently serving as multiple medical institutions
If a designated physician concurrently serves as multiple medical institutions and creates a “medical certificate / statement of opinion for the physically handicapped” at a medical institution, etc., a “concurrent post” is required.

If there is a change in the name of the doctor, the name of the medical institution, or the Address
If there is a change in the name of the designated physician, the name of the medical institution, or Address, a "Notification of Change" is required.

When a doctor dies, declines designation, or Retirement is a medical institution
If the designated doctor dies, declines, or retires from a medical institution, a "decline notification" is required.
