Article 15 of the Handbook for the Physically Handicapped?

A designated doctor based on the provisions of Article 15 of the Welfare Law for the Physically Disabled (hereinafter referred to as “Article 15 Designated Doctor”) is a doctor who creates the “Medical Certificate / Statement of Opinion” required for applying for a certificate of the physically disabled. That is.

Article 15 Designated doctors are designated by the prefectural governors (including designated city mayors and core mayors) after hearing the opinions of the Local Social Welfare Council according to the location of the medical institution.

In Yokohama-shi, we hold Yokohama-shi Council of Social Welfare obstacle degree examination sectional meeting four times a year (March, June, September, December) and make designation after hearing the opinion.

About procedure of designated application of Article 15 designated doctor

Doctors wishing to be designated should attach the necessary documents after filling out the following applications, etc., and submit it to the Yokohama City Rehabilitation Consultation Center by the deadline.

In addition, please note that if you are transferred to a medical institution in Yokohama, even if you have been designated outside Kanagawa Prefecture, you will need to apply for a new designation application.

Documents to be submitted

(1) Designated application form of doctor based on Article 15 of the Welfare Law for the Physically Disabled

(2) Career record
※Please fill in the name of the medical institution, medical treatment Families, work style (full-time and part-time), job title, clinical training period and name (initial clinical training, late clinical training, etc.) accurately.

(3) Copy of doctor's license

The deadline for submitting application documents is until the 10th of the previous month of the examination month (March, June, September, December).

 

Article 15 About transfer, decline of designated doctor

After receiving the designation, if there are any changes in the following, notification is required.

When a medical institution is transferred

If the designated doctor transfers a medical institution to which he belongs, a "transfer notification" is required.

When transferring a medical institution in Kanagawa Prefecture

Please submit "transfer notice" to each local government (prefecture, designated city, core city) before transference and local government (prefecture, designated city, core city) after transference.

When transferring to a medical institution outside Kanagawa Prefecture

Please submit "notice of transfer" to Yokohama-shi, and please apply for designation again to local government after transference.

※If you move from a medical institution outside Kanagawa Prefecture to a medical institution in Yokohama City, you need to specify a new designation in Yokohama City.
  

When concurrently serving multiple medical institutions

If the designated physician concurrently serves multiple medical institutions, etc., and also creates a “medical certificate / statement of opinion for the physically handicapped” at a medical institution that also serves as the same time, a “concurrent post notification” is required.

※This is a procedure for concurrently working at a medical institution in Kanagawa Prefecture.
  

Changes inAddress

If there is a change in the name of the designated doctor, the name of the medical institution or the Address, a "Notification of Change" is required.

When a doctor dies, declines designated, or Retirement of a medical institution

If the designated physician dies, declines the designated doctor, or retires from a medical institution, a "decree" is required.
