In Yokohama City, we carry out business to support communication at the time of hospitalization for people with severe disabilities.
In this project, when communication requires support, a communication support member will be dispatched to the hospital (excluding psychiatric hospitals and psychiatric wards) to facilitate smooth communication between the staff of the medical institution and the person. Is what you do.

1 Purpose of BusinessIf a person with severe disability is hospitalized and cannot fully communicate with the staff of the hospitalized medical institution, a communication support member will be dispatched to provide smooth communication between the person and the staff of the medical institution. To do.
2 Target personsChildren with disabilities and persons with disabilities who are elementary school students or older living in Yokohama City and who apply to all of the following (1) to (4). However, this does not apply to cases where disability support category 6 can receive communication support during hospitalization with severe home-visit care.

(1)Those who fall under any of a to c
Oh, person who acquired certificate of the physically disabled of limb dysfunction class 1.2 by reaching 65 years of age or equivalent
B. Those who have been judged to have intellectual disability
C. Those who have mental disorders by the age of 65

(3)Those who use disability welfare services, etc.
(4)Those who can obtain the consent of the medical institution where they are hospitalized
3 Communication Support Members

Helpers who are involved in daily support of the individual, staff at tsusho, staff at group homes, staff at entrance facilities (excluding nursing care establishments), staff at local activity homes, etc.
In order to support smooth communication between the person and the doctor / nurse, we support communication, such as communicating the person's chief complaint at the time of consultation or at the hospital room.
※Support that is subject to medical fees and shopping agency are not eligible.

4 Number of hours usedUp to 150 hours per hospitalization. (There is no limit on the number of days.)
※Plan and adjust the dispatch time, etc. within a range that does not exceed the limit between users and businesses.
5 User burdenNo. (If you are dispatched to a hospital outside the city, you may be required to pay for transportation expenses for dispatching support staff.)

1 Pre-registration (In the case of emergency hospitalization, it is also possible to register and use at the time of hospitalization.)
(1)If you would like to dispatch a support member, check with the business or facility you usually use to see if you can request a dispatch as a communication support member, and obtain consent.
(2)Once the consent of the dispatch is obtained, apply for registration at the ward Health and Welfare Center window in advance.
2 When hospitalized
(3)Get permission from the hospital for communication support staff to enter.
(4)Request the dispatch of communication support staff to registered businesses and facilities.
(5)Submit the “Notice of Start of Use” to Health and Welfare Center.
3 When discharged from the hospital
(6)Submit the “Notice of End of Use” to Health and Welfare Center.
