In Yokohama City, we carry out business to support communication at the time of hospitalization for people with severe disabilities.
In this project, when communication requires support, a communication support member will be dispatched to the hospital (excluding psychiatric hospitals and psychiatric wards) to facilitate smooth communication between the staff of the medical institution and the person. Is what you do.
1 Purpose of Business | If a person with severe disability is hospitalized and cannot fully communicate with the staff of the hospitalized medical institution, a communication support member will be dispatched to provide smooth communication between the person and the staff of the medical institution. 2 Target persons | Children with disabilities and persons with disabilities who are elementary school students or older living in Yokohama City and who apply to all of the following (1) to (4). However, this does not apply to cases where disability support category 6 can receive communication support during hospitalization with severe home-visit care. ※We confirm requirements in ward Health and Welfare Center along confirmation table (see attached table) (PDF: 227KB). (1)Those who fall under any of a to c Oh, person who acquired certificate of the physically disabled of limb dysfunction class 1.2 by reaching 65 years of age or equivalent B. 3 Communication Support Members | Helpers who are involved in daily support of the individual, staff at tsusho, staff at group homes, staff at entrance facilities (excluding nursing care establishments), staff at local activity homes, etc. | |
4 Number of hours used | Up to 150 hours per hospitalization. (There is no limit on the number of days.) ※Plan and adjust the dispatch time, etc. within a range that does not exceed the limit between users and businesses. | |
5 User burden | No. (If you are dispatched to a hospital outside the city, you may be required to pay for transportation expenses for dispatching support staff.) | |
6 Business Flow | Refer to the flow chart (PDF: 262KB) 1 Pre-registration (In the case of emergency hospitalization, it is also possible to register and use at the time of hospitalization.)
