Promoting comprehensive community care in Izumi Ward!

In order to respond to the rapid aging of the population in the future, medical care, nursing care, prevention, life support, etc. will be integrated so that the elderly can continue their own life in the area where they are accustomed to living until the end of their lives. We will build a comprehensive community care system that will be provided at each community care plaza.

Izumi Ward

Izumi Ward is expected to increase by about 1.4 times the population of elderly people aged 75 and over, by 2025, when the baby boomers are 75 years or older.
In addition, among elderly people aged 65 and over, the proportion of those who have received nursing care (support required) certification is also increasing.

In Izumi Ward, the population between the ages of 15 and 64 is expected to decrease by about 10% and the number of elderly people aged 75 and over will increase by about 45% by 2025 compared to 2015.

Changes in the number of certified The Long-term Care Insurance in Izumi Ward

Comparing the end of March 2012 with the end of March 2017, the number of certified The Long-term Care Insurance and the percentage of certified persons in The Long-term Care Insurance have increased. Further growth is expected to increase further toward 2025.

Izumi Ward Community Comprehensive Care System

Izumi Ward Action Guidelines for Establishing Yokohama-based Community Care System

Based on the characteristics of Izumi Ward from a medium- to long-term perspective, Izumi Ward has adopted the Izumi Ward Action Guidelines for the Construction of Yokohama-based comprehensive community care system (hereinafter referred to as the "Guidelines"). In fiscal year 2017, we formulated the plan.
While these guidelines are positioned in the administrative plan, it shows common goals and initiatives not only by the Izumi Ward Office but also the Izumi Ward Japan National Council of Social Welfare, Community Care Plaza, medical and nursing care offices. Based on these guidelines, we will promote the construction of the Izumi Ward version of community-based comprehensive care system in conjunction with the efforts of the Izumi Ward Community Health and Welfare Plan for the elderly, but also disabled and children.
In addition, this guideline shall supplement the Yokohama Community Comprehensive Care Plan (7th plan period of Yokohama City Elderly Health and Welfare Plan / The Long-term Care Insurance Business Plan: FY2018-32), and plans every three years This guideline will be updated as appropriate when reviewing.
