Person who received authorization of support required 1-2 makes care prevention care plan in local Elderly Care Management Center.
Local Elderly Care Management Centers in each region are located in the Community Care Plaza.

Those who have received nursing care 1-5
Those who wish to use the service at home
Determine a care manager and make a care plan.
