（１） Overview of group (style 3) (word: 19KB)
（２） Drawing of the midwifery office
（３） Brochures, etc. that show the business of the organization
（４） If the establisher is a corporation, the Articles of Incorporation
（５） The latest “On-site inspection based on the provisions of Article 25, Paragraph 1 of the Medical Law” result report, or a midwifery site survey report

（６） If you are listed on the Yokohama City Bidding Qualification List, present your registration number.
　　　If it is not listed, a municipal tax tax payment certificate.
