Those who are eligible to participate in this proposal are required to submit the proposal preparation guidelines pertaining to "Advanced Care Planning Spread Awareness Drama Production Business Outsourcing".
All of the conditions described in "4 Participation Conditions (1) or (2)" shall be satisfied and shall not apply to the limit of "4 Conditions (3)".

（１） Date and time: It is scheduled from 9:00 to 17:00 on Friday, April 9, 2021.
（２） Location: Meeting room in Yokohama City Hall (6-50-10, Honmachi, Naka-ku, Yokohama)

1 Procedures for Participation
Businesses wishing to participate in this proposal should submit one copy of the following documents.
（１） Application Form (Form 1) 1 copy
　※In the case of a joint proposal, submit one copy of the “Application Form for Participation (Joint Proposal (JV))” (Form 2)” together.
（２） 1 copy of the written oath (Form 3)
　※In the case of a joint proposal, submit one copy for each participating company.

2 Submission of Proposals
Please submit the following documents for those who have submitted the application form for participation and who have been qualified for the proposal.
In addition, please make in prescribed format, etc. based on the proposal preparation guidelines [Attachment 1] "Submission of Proposal".
（１） 1 copy of proposal (Form 7)
（２） Proposed documents 1 to 10 sets (paper output, double clipping each set)
（３） 1 copy of data (DVD-R) ※All data must be in PDF format.
　※Please refer to the "Business Explanation Materials" and "Documents on the direction of drama production" sent by this section to businesses that meet the proposal qualifications.
　※Please use the "Submission Checklist" when submitting each document.

Bring or mail
•If the submission deadline has passed, we will not accept it. However, if the arrival is delayed due to the delivery company's reason, it will be accepted with the proof.
•In the case of mailing, please write "Application for Participation / Pledge" and "Proposal is in progress" on the envelope. Also, be sure to contact the submission address by phone or e-mail after shipping.
•In the case of bringing your own, Medical Care Bureau Cancer and Disease Control Division Home Health Care Section from 9:00 am to noon and 1 pm to 5 pm on weekdays.

If you have any doubts about the contents of the implementation guidelines, etc., please submit a questionnaire (Form 4) as follows.
About question contents and answer, we notify all proposal submission requesters.
If you do not have any questions, you do not need to submit a questionnaire.

•We will confirm the proposal qualification of the applicant who intends to participate in this proposal, and mail a “Proposal Qualification Confirmation Result Notice (Form 5)” regardless of eligibility. If it is confirmed that you are eligible for the proposal, we will also send you a "materials on the direction of drama production" by e-mail.
•Notifications will be made by Monday, March 8, 2021.
•The applicant who has been notified that the proposal qualification has not been confirmed can request a written explanation of the reason why the proposal qualification was not confirmed. In addition, please submit document to City of Yokohama, Medical Care Bureau cancer, disease measures section by 5:00 pm five days after excluding city hall closing agency day from the next day when Motoichi sent notice.
•When explanation is requested by the preceding paragraph, we will reply in writing to person who requested explanation within five days except city hall closing day from the next day when Motoichi received document.
