When setting up specific facilities, it is necessary to submit a report based on the Water Pollution Control Law.
Please refer to the report style table for the requirements of each report.

Precautions for Preparation
Please note that we have reviewed a part of the style of notification of installation, use, change notification and abolition of use abolition from January 2024.
①seal is not required. (After December 28, 2020)
②If the notifier is a corporation, it is, in principle, the name of the representative of the corporation. When submitting registration form on behalf of the factory manager, etc., a power of attorney must be submitted. However, the legal responsibility lies with the representative.
③About drainage destination, please confirm in "public sewer common start area figure" of "daichan map" of Yokohama-shi administration map reporting system (outside site). （Confirmation method (PDF: 1,821KB)）
④Please refer to the description method in the table for the description of each report. In addition, about notification of installation, use, change notice, please see the description example.

⑥It is not possible to fill in with pencil or disappearing ballpoint pen.
⑦Please standardize registration form to A4 size as much as possible. For large-sized documents such as drawings, fold A3 size into A4 size.

Submission deadline
In the case of installation notification / change notification, construction related to installation and change is not possible for 60 days from the date of receipt of the notification.
However, construction can be started with a notice of shortening the implementation restriction period.
This notice will be issued in writing or electronic data (with an electronic signature).
The electronic signature uses the certificate of responsibility of the Local Government Organization Certification Infrastructure (LGPKI). Please use the "Public Personal Authentication Service (external site)" of the Local Government Information System Organization or the "SkyPDF Verification Service" of Skycom. (At this time, the certificate of responsibility is not registered in Adobe's AATL (Adobe Approved Trust List), so Adobe Acrobat displays "¥ at least one signature is a problem." Is valid.)
