○Response to questions and inquiries
Questions regarding the acquisition of treatment improvement addition, including the transition to the new addition, and the transition to the upper category will be accepted by e-mail or fax, and a social insurance attorney will answer.

①Documents to be submitted
From this year, presentation documents to Motoichi become two points of "report of addition (the situation list such as systems)" and "treatment improvement plan".
In Yokohama City, "Notification of addition (list of status such as systems)" was not required until now, but please submit by all means from this year.
②About submission deadline

[In the case of obtaining additional credits after June 2024 (new/change)]
<Home Service> Submission deadline: 15th of the previous month of the calculation start month (in the case of calculation on June 1, 2024, the submission deadline is May 15, 2024)
<Facility Services> Submission deadline: On the first day of the calculation start month (in the case of calculation on June 1, 2024, the submission deadline is June 1, 2024)
※Until FY2023, the submission deadline was the last day of the month before the calculation start month for new calculations, and the last day of the month before the change month in the case of change, but in FY2024 the submission deadline has been changed as described above.
