0.Notice (Changes from FY2023)

①Documents to be submitted

From this year, presentation documents to Motoichi will be two points: "Notification of addition (status table such as systems)" and "Treatment improvement plan".
In Yokohama City, "Notification of Addition (Status List, etc.)" has been unnecessary until now, but please submit it from this year.

②Deadline for submission

[When acquiring additional charges from April or May 2024 (new/continuation)]
　Submission deadline: Monday, April 15, 2024

[When acquiring additional credits after June 2024 (new/changes)]
　<Home Service> Submission deadline: 15th of the previous month of the month in which the calculation starts (in the case of calculation on June 1, 2024, the submission deadline is May 15, 2024).
　<Facility Services> Submission deadline: 1st day of the calculation start month (in the case of calculation on June 1, 2024, submission deadline is June 1, 2024)

※Until FY2023, the submission deadline was the last day of the month before the calculation start month, and in the case of a change, the submission deadline was the last day of the month before the change month, but in FY2024, the submission deadline has been changed as described above.
