About FY2023 treatment improvement addition (treatment, identification, base-up addition)

This page is dedicated to notification of treatment improvement addition for FY2023.
We will inform you of the Ministry of Health, Labor and Welfare regarding the 2023 care staff treatment improvement addition, the specific treatment improvement addition for care staff treatment improvement addition, and base up for care staff in 2023.
1.Treatment improvement addition plan notification form
This is an electronic application window for notification of new or change of treatment improvement addition (treatment, identification, base-up) in FY2023.
※Presentation documents to Motoichi are only "plan". It is not necessary to submit a list of conditions such as registration form or the system.
(In other local governments, a list of situations such as the system may be required, so please check the website of each designated right holder.)
Submission deadline (if a new addition is obtained)
・When acquiring additional credits after June 2023 (new): The last day of the month before the month in which calculation starts
・When acquiring additional credits from April or May 2023 (new) → Until 23:59 on Saturday, April 15, 2023
Submission deadline (when there is a change in some items of the plan submitted at the time of additional acquisition)
・The last day of the month before the month in which the addition is to be calculated based on the changed content

２. Documents to be submitted
※From fiscal 2023, "registration form on Special Circumstances" and "registration form for Changes" are attached after the plan form. Please fill in and submit only the offices that need to be submitted. (4. It is the same as the document attached to the form list.)
