This is an electronic application window for notification of new / change of treatment improvement addition (treatment / specific / base-up) in FY2023.

※Submission documents to Motoichi are only "plans". It is not necessary to submit a list of status such as registration form and systems.
(In other local governments, it may be necessary to list the status of the system, etc., so please check the websites of each designated right holder etc.)

Submission deadline (when a new addition is obtained)

・In the case of obtaining additional credits after June 2023 (new): Last day of the month before the calculation start month
・In the case of obtaining additional credits from April or May 2023 (new) →　Until 23:59 on Saturday, April 15, 2023

Submission deadline (when there is a change in some items of the plan submitted at the time of acquisition)

・The last day of the month before the month in which the addition is to be calculated based on the changed content
