Initiatives for a comprehensive community care system

Toward 2025, when all baby boomers will be 75 years old or older (late-elderly), the number of elderly people living alone, elderly couples only households, certified persons requiring nursing care, and people with dementia is expected to increase significantly. The population is expected to decrease. Medical care, nursing care, prevention, housing, and life support are integrated so that the elderly can respond to the increasing quality and quantity of living issues of the elderly and continue to live their own lives in the area where they live. Must be provided. That is a mechanism called "comprehensive community care system", and its construction is urgently needed.

In March 2018, Aoba Ward formulated the Aoba Ward Action Guidelines for the Establishment of a Comprehensive Community Care System for Yokohama, visualized its goals and action items, and promoted them in cooperation with each other based on a common understanding.
Based on changes in initiatives and circumstances since the formulation, we are proceeding with the revision of the Aoba Ward Action Plan for the Construction of a Yokohama-type comprehensive community care system while receiving opinions from everyone.
Under the Aoba Ward Action Plan, we have set up six themes: support for decision-making by residents of the ward, prevention of health and nursing care, life support by various entities (improvement of life support systems), medical care and nursing care cooperation, support for dementia, and senior social participation.

Efforts to build a comprehensive community care system
- Promote health promotion and nursing care prevention.
- It activates a little help around you.
- We aim to be an area where people can live with peace of mind even if they have dementia.
- We aim to be an area where elderly people can continue to play an active role.
- Expand cooperation between medical care and nursing care.
While there is a need for elderly people to continue living in the area they are accustomed to for a long time, it is predicted that they will face a super-aging society and will not be able to enter or enter the desired facilities. Cooperation between medical professionals and caregivers is essential to support elderly people who have both medical needs and nursing care needs in their home recuperation life. To this end, Aoba Ward has promoted multi-professional collaboration among medical professionals, such as the Medical Association, Dental Association, Pharmacists Association, and other medical professionals, as well as nursing care professionals.
It is also featured in the March 2018 issue of the public relations Yokohama Aoba Ward version.

Kick-off seminar for the construction of a comprehensive community care system for medical and nursing care cooperation [finished]
In order to respond to the rapidly increasing demand for medical care and nursing care, there is a need for a system in which local medical institutions and nursing care providers work together to support the medical care, nursing care and living of the elderly throughout the community.
In Aoba Ward, which is expected to rapidly age in the future, a kick-off seminar was held by medical and nursing care providers involved in home healthcare and care in the city.
