Submission of application form

Must arrive at 5:00 pm from Friday, June 26, 2015 to Tuesday, June 30, 2015 (postable)
(Tsuzuki Ward   Office, Hours: 8:45 a.m. to 5:00 p.m. excluding Saturday, holidays and public holidays)

Arrange the application documents in order from Form 1, attach this cover, and submit 14 copies (one of which is the original).

Establishment of Selection Committee
