In Yokohama-shi, one person with a disability community activity center is set up in each ward. Opportunities for those who have sequelae such as cerebrovascular disease (cerebral infarction, cerebral hemorrhage) living in the city, opportunities for interaction with friends with the same disease, light work, recreation, etc. Offers.

This is a leaflet of the Community Activities Center for People with Disabilities in Seya Ward.

1 This is the first step in increasing the number of out-of-hospital visits.

Rehabilitation class

In the rehabilitation classroom, those who have developed cerebrovascular disease, etc., have completed rehabilitation at the hospital, gather people who have just been discharged from the hospital, maintain and improve their physical condition while interacting with friends, and expand the scope of activities. We hold for the purpose.

(1) Target: Those who live in the city and are generally between 40 and 64 years old and can go to the venue by themselves

(2) Schedule: From 13:30 to 15:00 every Wednesday (about twice to 4 times a month, about 35 times / year, in principle 1 year)

(3) Venue: Futatsu Bridge Daini Community Care Plaza (469, Futatsubashi-cho, Seya-ku, Seyamaru, Fureaikan) and others
  ※The venue is subject to change depending on the program contents.

(4) Costs: Free of charge

(5) Contents: Light sports experience, language rehabilitation, music recreation, bus getting on and off training, etc. conducted by group

(6) Application Flow
  Step 1 Consultation
  If you would like to participate, please contact the City Office Elderly and Disabled Support Division Public Health Nurse. (Phone: 045-367-5716)
  Step 2 Visit
  Public health nurses and nurses at the ward office visit your home and talk about family members, such as the physical condition and living conditions of the target person, the progress of treatment. At that time,
  I would like to explain the contents of the rehabilitation class.
  Step 3 Deciding to participate
  We will inform you about participation in the rehabilitation class at a later date.

(7) Other supplementary items

  • Consultations are also accepted by supporters (care managers, hospital counseling staff, etc.) in addition to the target person and family.
  • Please be sure to obtain the consent of family doctor.
  • If the condition is not stable, we may refuse to use it.
  • Even if you refuse to participate in the rehabilitation class, please continue to consult if you have any questions about maintaining and improving your physical strength or have any problems in your life.

2 Activities with peers, as regular outings

One-Step Seya (Community Center for People with Disabilities)

For those who have sequelae due to cerebrovascular disease, etc., have regular opportunities to go out while living at home, and to live independently in the local community through creative activities, sports, health management, community exchanges, etc. It is a place to work for.

(1) Those who can use: Those who live in the city and are generally between 40 and 64 years old and can go to the venue by themselves
  ※As a general rule, it will be used after participating in the rehabilitation classroom, but please feel free to contact us.


(2) Venue: One Step Seya (1st floor, Sugimoto Building, 295-35, Futatsubashi-cho, Seya-ku)

(3) Date and time: From Monday to Friday from 10:00 to 15:00 (participation date and frequency of participation will be determined after consultation)

(4) Costs: 1,000 Yen/month

(5) Contents

  • Creative activities for independent products such as beads, craft tapes, and knitting
  • Sports (bocha, rolling, etc.)
  • Exchanges with friends and local communities, sale of independent products at local bazaars, etc.

3 I would like to interact with people with similar illnesses

4 For those who are having trouble with the aftereffects of stroke or brain trauma (special consultation for people with higher brain dysfunction)

Specialized consultation for people with higher brain dysfunction

Diagnosis, rehabilitation, daily contact, life and work ideas, social participation, etc. for symptoms such as `` many inadvertent mistakes '', `` anger '', `` easy to get tired '' and `` easy to forget '' caused by brain damage We respond to consultation.

(1) Target: Persons with symptoms such as lack of motivation, forgetfulness, and anger after a stroke or brain trauma and their families and support staff

(2) Location: One Step Seya (1st floor, Sugimoto Building, 295-35, Futatsubashi-cho, Seya-ku)

(3) Consultation date: Twice a month (reservation required) including the first Thursday afternoon (between 14:00 and 16:00).
   ※In addition to regular days, a separate consultation day is also provided. Please contact us as the schedule will be adjusted according to your convenience.
