Check ups for Infants and Toddlers

In ward office, we carry out infants medical examination of 4-month-old child, 1-year-old 6-month-old child, 3-year-old child.

Individuals will be sent an individual notice at least one month before the date of the medical examination.
Please contact Sakae Ward Office Children and Families Support Division (894-8410) if it is not convenient for the medical examination date indicated in the individual notice.
Infant health checkups are an opportunity to confirm the growth of your child at the milestone of growth and support child care, and it is important to have them take them at a better time.

※Due to the adjustment of the number of people, you may have passed the age of the medical examination day, or you may be notified on a consultation day different from the scheduled date.
If there is no particular problem, please come on the date of the medical examination that was announced in the notice. Thank you for your understanding and cooperation.

・Reception will be held in two hours. Please come to Reception hours where you showed me. Please note that the order may change depending on the situation.

・You will not be able to eat at the medical examination site. Medical examinations will begin immediately after the reception, so we recommend that you complete your meal in advance.

・If you feel unwell, please refrain from consulting. Please contact Sakae Ward Office Children and Families Support Division (894-8410) as if it is not convenient. We will inform you of the next and subsequent medical examination days.

・The parking lot is expected to be crowded, so if you come by car, please come with plenty of time.

・Thank you for your cooperation in taking out used diapers.

You can receive three free health consultations (medical examinations) using the “Medical Institution Check ups for Infants and Toddlers Consultation Card” in the separate volume of the Maternal and Child Health Handbook “Health Examination Ticket Spelling”.
(1st month after birth, 2nd 7 months after birth, 3rd 12 months after birth)
Children who fall under the age of the consultation slip should have a medical examination.
※Please use it within the validity period indicated on the consultation slip.
※As we do not tell about medical examination individually, please consult medical institution in Yokohama-shi within validity period by all means.
※We do not reissue consultation slips due to loss or other reasons.

In addition, please confirm beforehand as conduct date varies according to medical institution.
※Medical examinations are not available at medical institutions outside Yokohama.
※Reimbursement (refund) of expense when we consult outside the city is not possible.
