Service Content
A part-time physician (psychiatrist) will provide consultation through interviews with the person, family, and stakeholders. For those who have difficulty visiting the office, we also have a visit.

Service Content
Families gather to exchange information on nursing care and think about the caregiver's own health.
In the gathering of juvenile dementia, the person also participates and interacts.

SOS network for elderly people with dementia

Discover and protect as soon as possible when a person with dementia goes missing due to wandering (leave out of the house and loses his way), discover and protect it as soon as possible. It is a mechanism to cooperate with.

Pre-registration is possible. Pre-registering features and photos will make the discovery smoother.
Please print out the registration form, fill in the required items and register at Sakae Ward Health and Welfare Center or the Community Care Plaza in your town. Registration forms are also distributed at Sakae Ward Health and Welfare Center and Community Care Plaza. At the time of registration, please bring your photo (one face, 1 whole body).

We carry out "right advocacy business" to receive consultation such as life of elderly person and person with a disability or daily money management and support life of the person based on contract.

The details of the business are as follows.

  1. Consultation business (free of charge)
    For example, you are worried about managing money on a daily basis, or wanting to decide in advance about property management after the death of a parent, etc.
  2. Regular visits and money management services (paid)
    Regular visits by staff, deposit and savings, payment of utilities, etc.
  3. Property-related documents custody services (paid)
Deposits and savings passbooks, real estate rights certificates, etc. (stored in a safe bank of financial institutions)

Assistive devices are displayed (you can experience trial). We will advise you on how to choose equipment that is in trouble, remodeling houses, etc. Consultation on assistance and nursing care are also available.

We provide a variety of support to make good use of The Long-term Care Insurance and other services so that the elderly can continue living in the area where they live.
