What is a community-based comprehensive care system?
This is a system in which medical care, nursing care, life support and nursing care prevention are provided in an integrated manner, centered on housing, in order to continue their own life in an area where the elderly are accustomed to living. Construction is underway by 2025, when the baby boomers will be 75 years or older.
Action Plan for community-based comprehensive care system in Sakae-ku, Yokohama
The action plan was formulated in March 2022 by revising the "Sakae Ward Action Guidelines for Building a Yokohama-based comprehensive community care system" formulated in fiscal 2017, and incorporates specific initiatives and goals.
