About infant medical examination in ward office
Infant health checkups (4-month-olds, 1-year-olds, 6-month-olds, 3-year-olds) conducted at the ward office are opportunities to confirm the growth of children at the milestone of growth and support child care. It is important to receive it.
※Please be sure to check the date of your consultation in the individual notice.
※Please fill in the medical questionnaire enclosed with the individual notice and bring it on the day.

Information on medical examination schedules
- An individual notice (in principle, sent by the end of the month before the designated date) will be sent in advance.
- Please be sure to go beyond the target age.
- If you cannot come on the designated date indicated in the notice, please come to the medical examination date after the designated date.
Request for consultation
When visiting the office, please cooperate with the following points.
- If you have a cold, a fever of 37.5 degrees or higher, or an acute rash, please refrain from consulting.
- Please refrain from consulting not only for children who are subject to medical examinations but also for those who have symptoms such as fever or cough in their family.
- In order to avoid congestion in the venue, the medical examination hall should be provided by one guardian per target child.
- Thank you for your cooperation in taking out used diapers. Please bring a filth bag.
