• 70 years old → Present a proof of age (health insurance card, etc.) (advance procedures are not required)
• People in the social security Household → Present a medical treatment request certificate for holidays and nights (advance procedures are not required)　
• All persons on the same resident certificate are taxable only for the city / prefectural tax exemption or per capita rate of the previous fiscal year → Present the “Confirmation Form for Exemption from Patients from health check-up” issued by Health and Welfare Center (Please bring your own identification card (such as a health insurance card) before receiving the health check-up and apply at the Health Promotion Section (41st floor on the 4th floor). If you are going to apply for living together relatives, please bring something that can identify the visitor and the examinee.
• Those who are subject to the Chinese residual Japanese support benefit system → Present your identity verification certificate (advance procedures are not required.)

※We do not carry out imaging tests with X-rays or removing tartars. If you receive treatment, you will be charged for treatment at the counter separately from the health check-up consultation fee.
