• social security households → Present a medical treatment request card for holidays and nights (prior procedures are not required.)　
• All persons on the same resident certificate are exempt from city and prefectural tax in the previous fiscal year (FY2019) or only the per capita rate → Present the health check-up Confirmation Form issued by Health and Welfare Center (Please bring a copy of your identity (health insurance card, etc.) before receiving the health check-up, apply at Health Promotion Section (41st floor, 41st floor). If you are going to apply for a living together relative, please bring something that can identify the visitor and the examinee.

※We do not carry out image inspection or tartar removal using X-rays. If treated, treatment fee will be collected at the counter separately from the health check-up consultation fee.
